Günümüzde nefron koruyucu cerrahi, erken tanı yöntemleri ile böbrek tümörlerinin tedavisinde sıklıkla uygulanan bir yöntem olmuştur, teknolojik gelişmeler ile de bu tümörlere laparoskopik yaklaşım sıklıkğı artmıştır. Cerrahi sırasında (APN-LPN) geçici olarak renal arterin klemplenmesi ile böbrekte iskemi reperfüzyon hasarı oluşur bu durum özellikle soliter böbreği olan hastalarda ve residü parankimi sınırlı olan hastalarda nefrotoksisiteye bağlı sorunların (üre, cr yüksekliğine bağlı geçici diyaliz) ortaya çıkmasına neden olabilir. Özellikle soliter böbrekli hastalar başta olmak üzere, renal cerrahi sırasında iskemi reperfüzyona bağlı renal dokuda oksidatif hasar gelişebilecek tüm olguları bu hasarın komplikasyonlarından korumak amacı ile cerrahi öncesi, insan vücudunda sentezlenmeyen, tüketilen besinlerden elde edilen, karotenoid, non enzimatik antioksidan ajan olan likopen ile profilaksisi amaçlandı. Yöntem: Adnan Menderes Üniversitesi Veteriner Fakültesi Deney Hayvanları Laboratuvarlarında çalışmaya alınan ratların serum üre, cr, Na ve K değerlerine bakıldı, ratlar iki gruba ayrıldı. Bir grup rata cerrahi öncesi 24 saat ara ile toplam iki kez 4 mg/kg likopen gavaj yöntemi ile verildi. Daha sonra çalışmaya alınan tüm ratlara sağ nefrektomi uygulandı ve abdominal aorta 45 dakika klemplenerek sol böbrekte iskemi reperfüzyon hasarı oluşturuldu. Klemp açıldıktan 24 saat sonra tüm ratların serum üre, cr, Na ve K değerlerine bakıldı ve sol nefrektomi uygulandı. Ratlar sakrifiye edildi. Böbrek dokusunun yarısı patolojik inceleme için Adnan Menderes Üniversitesi Tıp Fakültesi Patoloji Anabilim Dalı'na diğer yarısı doku enzim seviyesinin tespiti için Adnan Menderes Üniversitesi Tıp Fakültesi Biyokimya Anabilim Dalı'na gönderildi. Bulgular: Kontrol grubu ratların, iskemi öncesi üre değeri ortalaması 57,3 ± 16,2 (34-84) mg/dL, iskemi sonrası üre değeri ortalaması 148,8±72,8 (46-229) mg/dL olarak saptandı, iskemi öncesi cr değeri ortalaması 0,45±0,083 (0,4-0,6) mg/dL, iskemi sonrası cr değeri ortalaması 1,17±0,97 (0,5-3) mg/dL olarak saptandı (sırasıyla p=0,046, p=0,027). Kontrol grubu ratların, iskemi öncesi Na değeri ortalaması 141,5±3,37 (136,7-145) mmol/L, iskemi sonrası Na değeri ortalaması 133,6±7,26 (122-143) mmol/L saptandı (p=0,028). Likopen grubu ratların iskemi öncesi ortalama üre değeri 61,2±16,9 (37-86) mg/dL, iskemi sonrası ortalama üre değeri 159±78,8 (28-241) mg/dL, iskemi öncesi ortalama cr değeri 0,45±0,055 (0,4-0,5) mg/dL ve iskemi sonrası ortalama cr değeri 1,37±0,87 (0,4-2,8) idi (sırasıyla p=0,046, p=0,046). Likopen grubu ratların serum Na seviyesinde iskemi öncesi ve sonrasında anlamlı fark saptanmadı. Kontrol grubu ratların patolojik skoru 2,17±0,41 (2-3) idi. Likopen grubu ratların patolojik skoru 1,5±0,55 (1-2) saptandı (p<0,05). Kontrol grubu ratların doku MDA seviyesi ortalaması, likopen grubu ratların doku MDA seviyesi ortalamasından daha yüksek saptandı (p>0,05). GSH-Px'in doku düzeyi ortalaması likopen alan grupta daha yüksek idi (p>0,05). Sonuç: Renal iskemi reperfüzyon sonrası dokuda nefrotoksisite ve doku hasarı oluştu bu hasarın bir göstergesi olan doku MDA seviyesi ve patolojik skor likopen alan grupta daha düşük seviyede idi. Likopen'in serum Na seviyesini iskemik hasara karşı koruduğunu saptadık. Tüm bu sonuçlar dahilinde likopen'in APN ve LPN'de oksidatif hasara karşı koruyucu olabileceği düşünüldü.
Today nephron sparing surgery with early diagnosis provides effective curative therapy for patients with localized renal cell carcinoma. With technological developments laparoscopic surgergy usually used on this tumors. On surgery (OPN-LPN) when renal arter temporary clemping renal ischemia-reperfusion injury occurs (temporary dialysis for patients dependent on üre and cr increased ). On this condition nephrotoxicity improved especially have a soliter kidney or determinate kidney parancime patients. The present study was designed to invastigate the possiple protective effects of lycopene against hypoksia induced renal demage. Lycopene is the one of the potent carotenoid antioxidant agent to take on foods which couldn't synthesis on human body. Material and Method: Twelve rats are included to study from Adnan Menderes University Veterinary Faculty Laboratories, serum üre, cr, Na and K levels are detected for all rats. Famale wistar rats were diveded into two groups of six rats in each one; first group served as control, the other group were treated two days of orally lycopene (4mg/kg per day) before surgery. All wistar rats were subjected to right nephrectomy and after abdominal aorta clamping for 45 minutes for ischemia reperfusion injury. After 24 hours blood samples for taken again analysis of serum üre, cr, Na and K levels. And done left nephrectomy for biochemical and histopatholigical evaluation on Adnan Menderes University Medical Faculty Biochemistry and Pathology Laboratories. Findings: Mean of control group pre ischemia üre levels was 57,3±16,2 (34-84) mg/dL, post ischemia üre levels was 148,8±72,8 (46-229) mg/dL (p=0,046). Mean of control group pre ischemia cr levels was 0,45±0,083 (0,4-0,6) mg/dL, post ischemia cr levels was 1,17±0,97 (0,5-3) mg/dL (p=0,027). Mean of control group pathological score levels was 2,17±0,41 (2-3), mean of lycopene group pathological score levels was 1,55±0,55 (1-2) (p<0,05). Mean of control group tissue MDA levels was higher than lycopene group (p>0,05). Mean of lycopene group tissue GSH-Px levels was higher than control group (p>0,05). In conclusion: Ischemia reperfusion induced oxidative stress and nephrotoxicity caused significant increases in pathological score. And elavated tissue MDA levels, serum üre, cr, Na levels. Post ischemia serum Na levels was protected on lycopene group. For all results a natural antioxidant lycopene might have protective effects against hypoksia induced nephrotoxicity and utilizable on OPN and LPN.
