Endotrakeal Entübasyon Yöntemlerine Hemodinamik Yanıtların Karşılaştırılması Amaç: Geleneksel laringoskopla yapılan entübasyon ile vokal kordları görmeden Laringeal maske airway (LMA) Fastrach ve LCD kamerası eklenerek vokal kordları görmeyi sağlayan LMA Ctrach ile yapılan entübasyonlar sonucu gelişen hemodinamik yanıtların ve komplikasyonların karşılaştırılması amaçlanmıştır. Yöntem ve Gereç: Çalışma prospektif randomize olarak düzenlendi. Genel anestezi altında elektif cerrahi operasyon geçirecek ASA I-II 18 yaş üstü 75 olgu 3 gruba ayrıldı. Grup L'da Macintosh laringoskopla, Grup F 'da LMA Fastrach''la, Grup C 'da LMA Ctrach''la endotrakeal entübasyon yapıldı. Preoperatif mallampati ve kalp atım hızı (KAH), sistolik kan basınçları (SAB), diastolik kan basınçları (DAB), ortalama arter kan basınçları (OAB) bazal degerleri kayıt edildi. İndüksiyondan sonra, entübasyondan bir dk sonra ve devamında 10 dk boyunca 2.5 dk aralıkla KAH, SAB, DAB, OAB, SpO2 ölçümleri kaydedildi. Entübasyon süresi ve endotrakeal tüpün kaçıncı denemede trakeaya yerleştirildiği kayıt edildi. Ayrıca entübasyon sonrası oluşan mukoza diş yaralanmaları ve operasyon sonrası oluşan boğaz ağrısı, ses kısıklığı gibi komplikasyonlar kayıt edildi. Bulgular: Kalp hızı, SAB, OAB, SpO2 bakımından gruplar arasında istatiksel olarak anlamlı fark bulunmadı. Grup L, Grup F, Grup C mallampatisi I olan hasta sayısı sırasıyla 8, 17, 11 olarak saptandı. Bu fark istatiksel olarak anlamlı bulundu (p< 0.03). Gruplar arasında entübasyon süreleri arasında istatiksel olarak anlamlı fark vardı (p< 0.001). Bu farkın laringoskop grubundan kaynaklandığı tespit edildi. Gruplar arasında entübasyonun birinci denemede gerçekleşme oranı Grup L, Grup F, Grup C sırasıyla 24, 15, 16 idi ve postopreatif morbidite arasında istatiksel olarak anlamlı fark bulunmadı. Sonuç: Çalışmamızda Ctrach ya da Fastrach'ın geleneksel laringoskopla yapılan endotrakeal entübasyona üstün olmadığını, entübasyon için daha uzun süreye ihtiyaç duyulduğunu söyleyebiliriz. Ctrach'da ekran olması ve görerek entübasyon yapılması Fastrach'a göre daha güvenli bir teknik olduğunu düşündürmektedir.
Comparing Haemodynamic Responses To Endotracheal Entubation Methods Objective an aim: We compared the haemodynamic responses and intubation time and intubation attempt to traditional laringoscopic intubation with LMA Fastrach which is developed for difficult airway, without seeing vocal cords and with LMA Ctrach which makes us able to see vocal cords with LCD monitor. Materials and Methods: The study was designed as prospectively randomized. The 75 patients who are ASA I-II, over the age of 18, who will be operated under general anaesthesia for elective surgery, divided into 3 groups. In the Group L, the intubation made with Macintosh laryngoscope, in group F with LMA Fastrach TM, in group C with LMA Ctrach TM. Preoperative mallampathy, heart rate, systolic blood pressure, diastolic blood pressure, mean arterial blood pressures' basal values are recorded. After the induction one minute after intubation and during 10 minutes for each 2,5 minutes heart rate, systolic blood pressure, diastolic blood pressure, mean arterial blood pressure, SpO2 are recorded. The intubation times and number of attemps at endotracheal intubation recorded. Also after the intubation we recorded the mucosal and tooth injuries and postoperative complications such as sore throat and hoarness. Results: There wasn't a statistical significantly difference between the groups according to heart rates, systolic blood pressure, mean arterial blood pressure. Number of patients whose mallampathies were I, were 8, 17, 11 by order. This difference was found statistically significant (p<0.03). The intubation times between groups were found statistically significant (p< 0.001). We found that this difference results from the laryngoscope group. Successful intubation at the first attempt between the groups were 24, 15, 16 order of Group F, Group C and there wasn't a statistically significant difference for postoperative morbivity. Conclusion: In our study, we found that Ctrach or Fastrach are not superior to endotracheal intubation which is done with traditional laryngoscope and also we need more time for intubation. The presence of monitor and intubation by seeing with ctrach seems to be more safety compared to Fastrach.
