Amaç: Onkoloji hastalarına uygulanan kemoterapötik ilaçlar malign hücreleri yok etmek
için iĢlev yürütürken aynı zamanda vücudun kendi savunma hücrelerine de zarar
vermektedir. Bu durumda ortaya çıkan nötropeni enfeksiyon açısından en riskli durumdur.
Onkoloji hastaları için enfeksiyon morbit ve mortal seyredebilmektedir. Bu çalıĢmada
nötropenik hastalarda üreyen mikroorganizmaların dökümünün ortaya konulması
amaçlandı.
Gereç ve Yöntem: ÇalıĢmamızda 01.01.2019 - 31.12.2019 tarihleri arasında Aydın Adnan
Menderes Üniversitesi Hastanesi EriĢkin Hematoloji Servisinde yatarak tedavi edilen,
nötrofil sayısı 500/mm³ altında olan veya 500-1000 mm³ arasında olup, 24-48 saat içinde
500 /mm³ ün altına düĢmesi beklenen hastalarda, tek seferde vücuttan alınan ateĢin 38.3°C
ve üstü veya bir saat süreyle ≥38.0°C olan ve bu sırada kan kültürü alınan hastaların kültür
sonucu retrospektif olarak değerlendirildi. Yapılan çalıĢma sonuçları SPSS programı ile
çözümlenmesi yapılıp istatistik değerlerle sunulmuĢtur.
Bulgular: ÇalıĢma süresinde 54‟ü (%60) erkek, 36‟sı (%40) kadın olan 331 febril
nötropenik atak geliĢen hastalar değerlendirildi. Bu hastalardan alınan kan kültürlerinde
123(%52,79) Gram pozitif ve 110 (%47,21) Gram negatif bakterileri ürediği saptandı. Gram
pozitif bakterilerden Koagulaz Negatif Stafilokok (%25,76) en çok üreyen bakteri olarak
saptandı. Escherichia coli (%23,60), Pseudomonas aeruginosa (%10,73), Corynebacterium
spp. (%9,01), Enterococcus faecium (%6,43), sıklık sırasına göre üreyen diğer
mikroorganizmalardı. Stafilokoklar da Methicilin dirençi ortalama (% 52,62) saptandı.
Karbapenem duyarlılığı en fazla E. coli de tespit edildi. P. aeruginosa Ġmipenem‟e (%92)
duyarlı iken, Meropenem‟e (%96) duyarlı olduğu tespit edildi.

Sonuç: ÇalıĢmamızda birbirlerine yakın olmakla birlikte, Gram pozitif bakterilerin daha
fazla ürediği saptandı. Gram pozitif üreyen mikroorganizmalardan kontaminan olma
olasılığı ön planda olan Koagulaz Negatif Stafilokok en çok üreyen mikroorganizma olarak
saptandı. Stafilokoklarda metisilin direncinin fazla olduğu ve Gram negatiflerden Klebsiella
pneumoniae da karbapenem dirençinin yüksek olduğu saptandı.
Aim: The chemotherapeutic drugs given to oncology patients function to destroy the
malignant cell, they also damage the body's own defense cells. The neutropenia that occurs
in this situation is the most risky for infection. Infection can lead to morbid and mortal for
oncology patients. In this study, it was aimed to reveal the breakdown of microorganisms
grown in neutropenic patients.
Material method: In our study, in patients with neutrophil count below 500/mm³ or
between 500-1000 mm³ and expected to decrease below 500/mm³ in 24-48 hours, who were
hospitalized in the Adult Hematology Unit of Aydın Adnan Menderes University Hospital
between 01.01.2019 and 31.12.2019, the culture results of patients whose blood culture was
taken at the same time when they had fever of 38.3°C and above or ≥38.0°C for one hour
were evaluated retrospectively.
Result: During the study period, 331 patients who developed febrile neutropenic attacks, 54
(60%) male and 36 (40%) female, were evaluated. It was determined that 123 (52.79%)
Gram positive and 110 (47.21%) Gram negative bacteria were grown in blood cultures
taken from these patients. Among Gram-positive bacteria, Coagulase Negative
Staphylococcus (25.76%) was found to be the bacteria that grew the most. Escherichia coli
(23.60%), Pseudomonas aeruginosa (10.73%), Corynebacterium spp. (9.01%), Enterococcus
faecium (6.43%) were other microorganisms that reproduced in order of frequency.
Methicillin resistance was found on average (52.62%) in staphylococci. Carbapenem
sensitivity was mostly detected in E. coli. While P. aeruginosa was sensitive to Imipenem
(92%), it was found to be sensitive to Meropenem (96%).
Conclusion: In our study, although they were close to each other, it was determined that
Gram-positive bacteria reproduced more. Coagulase Negative Staphylococcus, which is the

most likely to be contaminant among Gram-positive growing microorganisms, was
determined as the most growing microorganism. It was determined that methicillin
resistance was high in staphylococci and carbapenem resistance was high in Gram negative
Klebsiella pneumoniae.
