İKLİM DEĞİŞİKLİĞİNİN İNSAN SAĞLIĞINA ETKİLERİ VE SAĞLIK
SİSTEMİNİN İKLİM DEĞİŞİKLİĞİNE UYUMU: ULUSLARARASI

Amaç: Bu araştırma, iklim değişikliğinin insan sağlığına etkilerini ve ICD-11’e eklenme
gerekliliğini niteliksel kapsamda açıklamayı amaçlamaktadır. Araştırmanın birinci bölümü,
iklim değişikliğinin insan sağlığı üzerindeki etkilerini tek çatı altında birleştirmeyi ve genel bir
bakış açısı sunmayı amaçlamaktadır. Araştırmanın ikinci bölümü, iklim değişikliğinin sağlık
etkilerini ölçebilmek için farkı disiplinlerden uzman görüşleri alarak ve ICD-11 revizyonuna
eklenen tanı kodlarını inceleyerek ‘iklim değişikliği’ tanı kodunun ICD’ye eklenme
gerekliliğini bilimsel açıdan kanıtlamayı amaçlamaktadır.
Gereç ve Yöntem: İklim değişikliğinin insan sağlığı üzerindeki etkilerini tek çatı altında
birleştirmek amacıyla ulusal-uluslararası alan yazın veri kaynağı olarak değerlendirilerek, iklim
değişikliği sağlık etkileri diyagramlarını içeren sistematik literatür incelemesi hazırlanmıştır.
Hazırlanan inceleme ve yazar tarafından oluşturulan yarı yapılandırılmış soru formu, e-posta
aracılığıyla iklim değişikliği alanında çalışmalar yapan uzmanlara, hekimlere ve ICD veri
kodlaması bilen katılımcılara gönderilerek görüşmeler yapılmıştır. Elde edilen veriler
Microsoft Excel’de düzenlenmiş ve bilgisayar destekli nitel veri analizi için kullanılan
MAXQDA programının 2020 sürümü ile içerik analizine tabi tutulmuştur.
Bulgular: Araştırmanın birinci bölümünde, sistematik literatür incelemesi kapsamında
172 çalışma, sekiz konu başlığı altında incelenmiştir. Araştırmanın ikinci bölümündeki yarı
yapılandırılmış görüşmelere 26 uzman katılım sağlamıştır. Katılımcıların 14’ü erkek, 12’si
kadındır. Katılımcıların yaş ortalaması 47,57’dir. Katılımcılardan 12’si iklim değişikliği
alanında çalışan akademisyenlerden, 11’i iklim değişikliğinin neden olduğu hastalıkların branş
hekimlerinden, üçü de ICD veri girişi yapan sağlık çalışanlarından oluşmaktadır. Katılımcıların
iklim değişikliğinin neden olduğu-olabileceği sağlık etkilerini yönetmede sorumlu sektörlerin
tespit edilmesine, uyum ve direnç artırmada rol alabilecek sağlık hizmeti sunucuları ve iklim
değişikliğinin sağlık etkilerinin ICD-11 kapsamına alınma gerekliliğine yönelik görüşleri

alınmıştır. Araştırma konusu, fen, sosyal ve sağlık bilimlerini kapsadığı için disiplinler arası bir
çalışma gerektirmiş, bu da zaman zaman tek bir disiplin üzerinde çalışma yapan katılımcıların,
farklı bir disiplinle ilgili görüş bildirirken çekimser davranmasına neden olmuştur ancak
katılımcı görüşleri arasında genel bir fikir birliği bulunmaktadır.
Sonuç: İklim değişikliği, sağlık ve ICD üçgeninde yapılan incelemeler ve uzman görüşleri
sonucunda, iklim değişikliğinin sağlık etkilerine uyum ve izleme noktasında ICD-11 revizyonu
‘X Uzantı Kodları’ ana başlığı, ‘Etiyoloji-Nedensellik’ alt başlığına eklenmesi gerektiği
kanısına varılmıştır. Daha önce DSÖ’ye ICD-11’e uzantı kod olarak ‘iklim değişikliği’ kodunu
ekleme önerisi sunulmuş ancak bu alanda yeterli bilimsel anlaşma olmaması nedeniyle teklif
reddedilmiştir. Bu çalışma iklim değişikliği, sağlık ve ICD ile ilgili yapılan ilk çalışma olma
özelliğini taşımaktadır. İklim değişikliği ve sağlık ilişkisini araştıran çalışmaların
metodolojilerinde; A iklim değişikliği etkisinin insanlar üzerinde X, Y, Z sağlık etkileri
olabileceği düşünülerek belirlenen tarih aralıklarında sağlık kurumları tarafından kodlanan X,
Y, Z tanı kodları ile A iklim değişikliği etkisinin karşılaştırılmasına sıklıkla rastlanmaktadır.
Bu durum, olası A iklim değişikliği etkisinin araştırmacı tarafından tahmin edilmemiş diğer
sağlık etkilerinin göz ardı edilmesine, belki de asıl ilişkinin kurulamamasına neden
olabilmektedir. ICD’ de uzantı kodları, kök kodlara ek bilgiler eklemede standardizasyon
sağlamak için oluşturulmuştur. Uzantı kodlarına iklim değişikliği kodu eklenirse insan sağlığı
üzerinde iklim değişikliğinin etkisi olduğuna dair bilgilerin kodlanabileceği bir alan yaratılmış
olur. Ortaya çıkan sağlık etkilerinin iklim değişikliği ‘nedeniyle’ gerçekleştiğinin bilinmesi,
uzantı kod olarak yer alması ile daha nitelikli verilerin elde edilebileceği ve daha somut
kanıtlara ulaşılabileceği düşünülmektedir. Ayrıca bu durumda hekimlerin çevre öyküsü alması
gerekecek, sosyal kanıt havuzu oluşturulabilecek, iklim değişikliğinin sağlık etkileri daha
izlenebilir hale gelecektir. Yapılan uzman görüşmeleri sonucunda, ICD-11 revizyonuna iklim
değişikliği kodunun eklenmesi ve iklim değişikliği-sağlık ilişkisine verilmeyen önemin artması
gerektiği, iklim değişikliği-sağlık ilişkisiyle ilgili başta sağlık çalışanları olmak üzere herkese
eğitimler verilmesi, konuyla ilgili sektörler/disiplinler arası çalışmaların ve farkındalık
çalışmalarının artırılması gerektiği sonucuna varılmıştır
IMPACTS OF CLIMATE CHANGE ON HUMAN HEALTH AND HEALTH SYSTEM
ADAPTATION TO CLIMATE CHANGE: INTERNATIONAL CLASSIFICATION OF
DISEASES (ICD)

Aim: This research aims to explain the effects of climate change on human health and
the necessity of adding it to ICD-11 in a qualitative context. First part of the research aims to
combine the effects of climate change on human health under one roof and to provide an
overview. The second part of the study aims to scientifically prove the necessity of adding the
'climate change' diagnostic code to the ICD by taking expert opinions from different disciplines
and examining the diagnostic codes added to the ICD-11 revision in order to measure the health
effects of climate change.
Materials and Methods: In order to combine the effects of climate change on human
health under a single roof, a systematic literature review with climate change health effects
diagrams were prepared by evaluating the national-international literature as a data source. The
prepared review and the semi-structured questionnaire created by author were sent via e-mail
to experts working in the field of climate change, physicians and participants who knew ICD
data coding and interviews were conducted. The data obtained were organized in Microsoft
Excel and subjected to content analysis with the 2020 version of the MAXQDA program used
for computer-assisted qualitative data analysis.
Results: In the first part of the study, 172 studies were examined under eight subject
headings within the scope of systematic literature review. 26 experts participated in the semistructured interviews in the second part of the study. 14 of the participants were male and 12 of
them were female. The average age of the participants is 47.57. Twelve of the participants are
academicians working in the field of climate change, 11 are branch physicians of diseases
caused by climate change, and three are health professionals who make ICD data entry.
Opinions of the participants were received regarding the identification of the relevant sectors
in managing the health effects caused or likely to be caused by climate change, the health

service providers that can play a role in increasing adaptation and resilience, and the necessity
of including the health effects of climate change in the scope of ICD-11. Since the research
topic covers science, social and health sciences, it required an interdisciplinary study, which
sometimes caused the participants who worked on a single discipline abstaining from
expressing their opinions about a different discipline, but there is a general consensus among
the participant views.
Conclusion: As a result of the examinations and expert opinions in the triangle of climate
change, health and ICD, it was concluded that the ICD-11 revision should be added to the main
title 'X Extension Codes' and sub-title 'Etiology-Causation' in terms of adaptation and
monitoring of the health effects of climate change. Previously, a proposal was submitted to
WHO to add the 'climate change' code to ICD-11 as an extension code, but the proposal was
rejected due to the lack of sufficient scientific agreement in this area. This study holds the
speciality of being the first study on climate change, health and ICD all together. In the
methodologies of studies investigating the relationship between climate change and health;
Considering that the A climate change effect may have X, Y, Z health effects on humans, it is
frequently encountered that the X, Y, Z diagnostic codes coded by the health institutions and
the A climate change effect are compared in the date ranges determined. This situation may
lead to ignoring of possible A climate change impact, other health effects that were not
estimated by the researchers, and perhaps the failure to establish the actual relationship between
these two.. Extension codes in ICD were created to provide standardization in order to adding
additional information to root codes. If the climate change code is added to the extension codes,
a space is created where information on the impact of climate change on human health can be
coded. It is thought that more qualified data can be obtained and more concrete evidence can
be obtained by knowing that the resulting health effects occur 'because of' climate change and
including it as an extension code. In addition, in this case, physicians will need to take an
environmental history, a social proof pool will be created, and the health effects of climate
change will become more traceable. As a result of the expert interviews, following subjects
were concluded that the climate change code should be added to the ICD-11 revision and the
importance currently not given enough to the climate change-health relationship should be
increased, trainings about the climate change-health relationship should be given to everyone,
especially healthcare professionals and the related sectoral/interdisciplinary studies and
awareness studies should be increased
