Türkiye'de kamu sağlık hizmetlerinde 1980 sonrası uygulanan neo-liberal ekonomi politikalarının sağlık personeli istihdamı üzerindeki etkileri: Sözleşmeli sağlık personeli istihdamının eleştirel bir değerlendirmesi

Abstract

Bu tezin amacı, Türkiye'de 1980'lı yıllardan itibaren uygulanan neo-liberal sağlık politikalarının yardımcı sağlık personeli üzerine etkilerinin araştırılmasıdır. Bu çalışma yapılırken birinci bölümde sağlık hizmetlerinin genel tanımı, dünyada uygulanan sağlık sistemleri kısaca incelenmiş ve ülkemizin sağlık sisteminin kategorize edilmesine yönelik ön araştırma yapılmıştır. Yine çalışmanın birinci bölümünde iktisat teorisinin geçirdiği evrelerde irdelenmiş ve bunun sağlık sistemiyle olan ilişkisi üzerinde durulmuştur. Çalışmanın ikinci bölümünde ise Türkiye'deki sağlık sisteminin Cumhuriyet devriminden günümüze geçirdiği evrim üzerinde durulmuştur. Cumhuriyet devrimiyle birlikte sağlık alanında yapılan uygulamalar Türk modernleşmesinin bir parçası olmuştur. Sağlık hizmetlerinin gelişim evreleri aynı zamanda Türk modernleşmesinin gelişim evreleriyle paralel bir çizgi taşımaktadır. Çalışmanın üçüncü ve son bölümünde 1980 sonrası uygulanan sağlık politikalarının personel istihdam modeline etkisi üzerinde durulmuştur. Sözleşmeli sağlık personeli istihdamına yönelik olarak hazırlanan soru formu İzmir ilindeki kamu hastanelerinde çalışan sözleşmeli sağlık personeliyle yüz yüze görüşülerek yapılmıştır. Toplanan veriler, çalışmanın ileri sürdüğü hipotezlerle karşılaştırılarak sonuçlara ulaşılmıştır. Sözleşmeli istihdam modelinin sağlık personeli üzerinde oluşturduğu etkiler ortaya konulmaya çalışılmıştır.

The purpose of this thesis is to do a research on the effects of Neo-Liberal health policies after 1980s on assistant health personnel. While doing this research, a general definition of health service is done in the first chapter and health systems applied around the world were examined. Another preliminary work has been done on how to categorize the health system in Turkey. The evolutions of economics have been analysed and the relationship between this and health systems have been pointed out in the first chapter as well. In the second chapter of the study emphasizes the evolution of health system in Turkey from the Turkish republic period to today. Together with the republic revolution, the practise on health care has been a part of modernisation of Turkey.The development stages of health services are at the same time parallel to the development of the Turkish modernisation. In the third and last chapter includes the study on the effects of health policies after 1980s on personnel empleyment model. The questionaire prepared on the employment of the contract personnel was done by face to face interviewing the contract personnel working at publis hospitals in İzmir. The results were taken by the data compared to the hypothesis. The effects of cotract employment model on health personnel have been analysed.
