Abstract: ÇEŞİTLİ KAYNAKLARDAN İZOLE EDİLEN STAPHYLOCOCCUS AUREUS’UN BAZI VİRULENS ÖZELLİKLERİNİN BELİRLENMESİ Kızanlık Koçak P. Aydın Adnan Menderes Üniversitesi Sağlık Bilimleri Enstitüsü Besin Hijyeni ve Teknolojisi Programı Doktora Tezi, Aydın, 2019 Staphylococcus aureus (S. aureus) ülkemizde ve dünyada gıda intoksikasyonlarında ve antibiyotiklere göstermiş olduğu direnç ile halk sağlığını tehdit eden patojenler arasında ilk sıralarda yer almaktadır. Hayvansal gıdaların S. aureus ile kontaminasyonu; doğrudan bulaşmış gıdaların elde edildikleri hayvanlardan kaynaklanabileceği gibi, dolaylı olarak da gıdaların işlenmesi süresince yetersiz hijyen şartlarına bağlı alet-ekipman ve personel kaynaklı şekillenebilmektedir. Metisilin Dirençli S. aureus (MRSA) başlangıçta nozokomiyal bir etken olarak bilinirken, daha sonrasında etkenin toplum ilişkili ve son olarak da çiftlik hayvanları ilişkili suşlarının ortaya çıkması halk sağlığı açısından oldukça büyük bir risk olarak değerlendirilmektedir. Bütün bu nedenler göz önünde bulundurularak, çalışmada çeşitli kaynaklardaki S. aureus düzeyi, etkenin bazı virulens özellikleri ve antibiyotik direnç profilinin saptanması amaçlanmıştır. Araştırma kapsamında 2016 ile 2018 yılları arasında Batı Ege Bölgesi illerinde (Aydın, İzmir, Muğla) bulunan çeşitli noktalardan temin edilen gıda (850 adet), mezbahada kullanılan alet-ekipman (150 adet bıçak, 150 adet masat ve 150 adet önlük) ile çalışan personelden (150 adet) olmak üzere toplam 1450 adet örnek incelenmiştir. Örneklerden konvensiyonel olarak tespit edilen 106 izolatın 92’sinin (%86,8) PCR ile doğrulaması yapılmıştır. Tank sütü örneklerinin %10,8’inin ortalama 3,01 log kob/ml, tulum peyniri örneklerinin %17’sinin ortalama 3,08 log kob/g, tavuk eti örneklerinin ise %12’sinin ortalama 2,89 log kob/g ve karkas örneklerinin ise %4’ünün ortalama 1,28 log kob/cm2 düzeyinde S. aureus ile kontamine olduğu belirlenmiştir. Mezbahada uygulanan kesim proses zamanı göz önünde bulundurularak prosesin başında alet-ekipman ile personelden alınan örneklerin hiçbirinde S. aureus’a rastlanılmamıştır. Prosesin ortasında alınan önlük örneklerinden 1’inde (0,58 log kob/cm2), personelin ellerinden alınan örneklerin ise 2’sinde (1,26 log kob/cm2) S. aureus saptanmıştır. Prosesin sonunda personelden alınan örneklerde S. aureus tespit edilememiş, ancak bıçak (0,81 log kob/cm2), masat (0,63 log kob/cm2) ve önlük (0,58 log kob/cm2) örneklerinden her birinden 1’er adet olmak üzere kesim prosesinde kullanılan alet-ekipmanlarda farklı düzeylerde S. aureus kontaminasyonu belirlenmiştir. S. aureus izolatlarında enterotoksin oluşturan 10 genin varlığı (sea-see, seg-selj, sep) varlığı araştırılmış ve 39 adet (%42,4) izolatın bir ya da birden fazla enterotoksin geni içerdiği belirlenmiştir. Enterotoksijenik izolatlarda, klasik enterotoksinleri oluşturan genler içerisinde en çok sed geni saptanmış, yeni tanımlanan enterotoksin genlerinden en çok sei (21 adet), seg (19 adet) ve seh (20 adet) genleri tespit edilmiş, bununla birlikte izolatların hiçbirinde selj geni saptanamamıştır. Ayrıca, S. aureus izolatlarının antibiyotiklere karşı duyarlılığı VITEK 2 sistemi ile incelenmiş, 92 adet izolattan 66’sının (%71,7) farklı antibiyotik gruplarına karşı dirençli olduğu belirlenmiştir. İzolatların en çok %65,3 oranında penisiline dirençli olduğu tespit edilmiştir. Bu düzeyi %22,8 ile oksasilin, %18,4 ile klindamisin, %11,9 ile eritromisin, %9,8 ile sefoksitin, siprofloksasin ve fusidik asit, %7,7 ile daptomisin, %6,6 ile tetrasiklin ve fosfomisin dirençliliği izlemiştir. Gentamisine karşı 1 adet (%1), teikoplanin, vankomisin ile trimetoprim/sulfametoksazole karşı 3’er adet (%3,2), linezolide karşı ise 4 adet (%4,3) izolatın dirençli olduğu belirlenmiştir. Ancak tigesikline karşı dirençli S. aureus izolatı saptanamamıştır. Çalışma kapsamında tüm izolatlarda PCR yöntemi ile mecA geni varlığı araştırılmış, 13 izolatta (%14,1) mecA geni tespit edilmiştir. Ayrıca mecA geni taşımayan izolatlar mecC geni varlığı yönünden incelenmiş, ancak izolatların hiçbirinde mecC geni varlığı belirlenmemiştir. Sonuç olarak, çeşitli kaynaklardan elde edilen S. aureus izolatlarının çoğunun enterotoksijenik genlere sahip olması, S. aureus kaynaklı gıda intoksikasyonlarının meydana gelmesinin mümkün olabileceğini, izolatlarda saptanan antibiyotik dirençliliğinin belirli antibiyotikler açısından çok yüksek olmasının ve aralarında MRSA izolatlarının da bulunmasının halk sağlığı açısından sorun yaratabileceği ortaya konulmuştur.
DETERMINATION OF SOME VIRULENCE FEATURES OF STAPHYLOCOCCUS AUREUS ISOLATED FROM VARIOUS SOURCES Kızanlık Koçak P. The contamination of S. aureus in food occurs due to the animal as the main sources of animal originated food, lack of hygiene during the processing stages, e.g. contamination of equipment and food-handler. Methicillin Resistant S. aureus (MRSA), initialy, was known as a nosocomial agent. Then community-associated MRSA, began to cause infections outside the health-care environment. The last significant emergence of MRSA has been associated with livestock animals. Due to this extended spectra in the dissemination, MRSA has become a great risk for public health. That is because of these reasons, this study was aimed to investigate the level of S. aureus in various sources and to determine some virulence features, and antibiotic resistance profile of this agent. In this study, a total of 1450 samples obtained from food (n:850), equipment (n: 150 knife, n: 150 knife sharpener and n: 150 apron) and workers (n: 150) at the slaughterhouse collected from some provinces (Aydın, İzmir, Muğla) located in Western Aegean Region of Turkey. Ninety-two out of 106 (86.8%) conventionally isolated isolates were confirmed as S. aureus by PCR. It was determined that 10.8% of bulk tank milk samples were contaminated with a mean of 3.01 log cfu/ml S. aureus. The contamination rates and mean contamination levels for tulum cheese, chicken meat and beef carcasses were 17% and 3.08 log cfu/g, 12% and 2.89 log cfu/g, and 4% and 1.28 log cfu/g, respectively. When the processing period duration considered, none of the samples taken from equipment and personel examined at the beginning of process were found to be contaminated with S. aureus. One of the samples taken from butcher aprons (0.58 log cfu/ cm2) and 2 of the hands of butchers (1.26 log cfu/cm2) were found to be contaminated when samples were taken at the middle of the processing period. At the end of the process, none of the samples taken from personel were found to be contaminated with this agent. But, one of the samples (each) taken from knives (0.81 log cfu/cm2), knife sharpeners (0.63 log cfu/cm2) and aprons (0.58 log cfu/cm2) were found to be contaminated with S. aureus. The presence of 10 gens (sea-see, seg-selj, sep) expressing enterotoxins in S. aureus were investigated and 39 of the isolates (42.4%) were found to be consisting one or more enterotoxin gens. In isolates, sed was the most often found classical enterotoxin producing gene, and sei was the most often found (21 isolates) new enterotoxin gene which was followed by seg (19 isolates) and seh (20 isolates). No selj gen was found in any of the isolates determined. The antibiotic sensitivity of S. aureus isolates were investigated by VITEK 2, and 66 out of 92 isolates (71.7%) were found to be resistant to various antibiotics. The isolates were mostly resistant to penicillin (65.3%), which was followed by oxacillin (22.8%), clindamycin (18.4%), erythromycin (11.9%), cefoxitin, ciprofloxacin and fusidic acid (9.8%), daptomycin (7.7%), tetracycline and phosphomycine (6.6%). Beside this, 1 isolate for gentamicin (1%) and 3 isolates for each of teicoplanin, vancomycin, trimethoprim/sulfamethoxazole (3.2%), and 4 isolates for linezolid (4.3%) were found to be resistant. But, no S. aureus was found to be resistant to tigecycline. In this study PCR was carried out for all isolates in order to determine mecA, and this gen was found in 13 isolates (14.1%). The isolates considered as not carrying mecA were subjected to PCR analysis in order to determine mecC. But no mecC was determined in any of the isolates analysed. It was concluded that most of the S. aureus isolates isolated from various sources related with food and food processing possesed enterotoxin genes which might have the possibility of causing foodborne intoxications, and high antibiotic resistance profiles of these strains, including MRSA strains, for some antibiotics might also result in public health hazards.
