Abstract: Renal Hücreli Karsinom (RCC) erişkinlerde en sık görülen renal tümörlerdir. Patolojik evre ve nükleer derecenin en önemli prognostik belirleyiciler olduğu bilinmekle birlikte, RCC'lerde izlenen yaygın heterojenite nedeniyle, aynı evreye sahip hastaların sürvileri arasında bile önemli farklılıklar bildirilmektedir. Bu nedenle RCC'lerde yeni prognostik göstergelerin bilinmesine ihtiyaç duyulmaktadır. Çalışmamızda RCC'lerde proliferasyon, apoptozis ve anjiogenezis gibi çeşitli faktörlerin bilinen prognostik faktörlerle ve birbirleriyle ilişkisi araştırılmıştır. Toplam 40 RCC olgusuna ait parafin bloklara gömülmüş doku örnekleri çalışmaya alındı. Olgulara ait, yaş, cins, histolojik tip, Fuhrman derecesi, TNM evresi, bilgileri kaydedildi. İmmunhistokimyasal yöntemle p53, Ki-67, CD34, bcl-2, EGFR, VEGF ve NF-'B boyaları uygulandı. Tüm immunhistokimyasal göstergelerin bilinen prognostik faktörler, survi ve birbirleri ile olan ilişkileri istatistiksel olarak değerlendirildi. Sonuçlarımıza göre; 40 olgunun 32 tanesi berrak hücreli, 6 tanesi papiller, 2 tanesi kromofob hücreli RCC idi. Hastaların ortanca izlem süresi 12,5 aydı. NF-'B (r: 0.448, p=0.004) ile p53 (r: 0.410, p=0.009) bcl-2 (r: 0.509, p=0.001), VEGF (r=0,516, p=0,001), EGFR (r: 0.410, p=0.009) arasında anlamlı ilişki saptandı. Kullanılan immunhistokimyasal göstergelerden hiçbirinin surviyi göstermede etkili olmadığı, çalışılan parametrelerden sadece tümör boyutunun bağımsız prognostik faktör olduğu bulundu. Yaptığımız çalışma sonucunda, yüksek damarlanma özelliğine sahip olan ve kemoterapötik direnci bilinen RCC'lerde surviyi gösteren yeni prognostik bir faktör belirlenememiştir. Ama çalışmamız, NF-'B gibi son yıllarda yoğun bir şekilde araştırılmaya başlanılan bir molekülün RCC'lerdeki fonksiyonu konusunda yeni bilgiler vermiştir. RCC'lerde üzerinde az bilgi bulunan NF-'B gibi çeşitli moleküllerin bu tümörlerdeki etkilerinin belirlenmesi ve çeşitli faktörlerle ilişkilerinin gösterilmesi RCC tedavisinde yaşam süresini etkileyen prognostik bir faktör olarak yeni gelişimler sağlayacaktır.
Renal Cell Carcinomas ( RCCs ) are the most commonly seen renal tumors in adults. Although patological stage and nuclear degree of the tumors are considered as the most important prognostic factors, due to the heterogenity seen in RCCs, there might be seen different survival rates among the patients who have the same stage. For this reason, we need to find out new prognostic factors in RCCs. In our research, we've investigated different prognostic factors such as proliferation, apoptosis and angiogenesis and their interactions with each other. All 40 RCC specimens have been worked in paraffin embedded tissues. The informations of patients such as age, sex, histological types, Fuhrman's grade, TNM stage have been recorded. p53, Ki-67, CD34, bcl-2, EGFR, VEGF, and NF-KB stains have been performed immunohistochemically. Known prognostic factors, survivals and their interactions of all immunohistochemical indicators have been evaluated statistically. According to our findings in 40 specimens, 32 clear cell, 6 papillary cell and 2 chromophobic cell RCC were observed. Mean survival rate of the patients was 12,5 months. Significant relationship has been determined between NF-'B with p53 (r: 0,410, p=0,009) bcl-2 ( r:0,509,p=0,001), VEGF ( r:0,516, p=0,001), EGFR ( r:0,410, p=0,009 ). None of the immunuhistochemical indicators has been found to be effective to show survival rates. Among the parameters worked on, only the size of the tumor has been shown to be an independent prognostic factor. According to the results of this study, we have not been able to find out a new prognostic factor to show survival rates in RCCs which had been know with a high vascularization and and chemoterapeutic resistance. However, our results for NF-'B which has been recently studied extensively have given some new findings in terms of its function in RCCs. Determinig the effect of various molecules such as NF-'B, with limited information in RCCs, and demonstrating its relation with different factors will provide new developments in RCC management in terms of a prognostic factor effective on survival.
